OSF Saint Anthony conducting survey on community health

ROCKFORD (WREX) -

OSF Saint Anthony Hospital is asking the public for its feedback on the state of health in the community.

The hospital will then use the information found from the Community Health Needs Assessment survey to adopt an implementation strategy to better meet the community's health needs. It is asking residents from Winnebago, Boone, DeKalb, and Ogle counties to participate.

Data will be collected through written and online surveys, as well as personal interviews and focus groups. To participate, click here. The survey will run from December 8 to December 15.
